DIGITAL SLIDES REASSURE PATIENTS BEFORE MOHS SURGERY

At a Glance

West Valley Dermatology in West Valley, Utah, concentrates on improving the health, function, and appearance of patientsskin through medical and surgical dermatology. Jesse Jensen, DO, a board-certified dermatologist and a fellow member of the American Society of Mohs Surgery, owns and operates the practice. Patients in West Valley have come to rely on the practice for Mohs micrographic surgery, minimally invasive treatments, and cosmetic work.

“The digital slides are useful tools that help Mohs surgeons illustrate biopsy findings for patients who want to see a visual confirmation of their condition. A picture’s worth a thousand words. You can explain all of the science in the world, but when a patient sees a digital slide, they know it.”

—Jesse Jensen, DO
West Valley, UT
solution

The Challenge

Dr. Jensen performs Mohs surgery between one and two days every week. After many of his patients with positive biopsies and cancers saw healed areas, they found it difficult to understand the need to undergo additional surgical procedures. Instead, many opted for watchful waiting and allowed critical months to pass.

Meanwhile, the staff at West Valley Dermatology found they were wasting time checking the fax queue and following up with medical assistants and pathologists to chase down pathology reports. In addition, sometimes details didn’t make it to the note that should have reached the pathologist.

The Solution

The PathologyWatch slide viewer gives Dr. Jensen instant access to his patients’ digital slides. Instead of relying solely on words to describe their condition, he now displays slides on a tablet to help his patients visualize the histology and see the difference between their tumor cells and normal cells. He says, A large majority of patients go forward with treatment feeling much more emotionally secure and trusting their physician made the right decision for them based on having a picture.”

At the same time, PathologyWatch integrated with the West Valley Dermatology EMR system, which imports results directly from the dermatopathologist. Now when Dr. Jensen is with a patient, he can pull up biopsy results in the EMR without leaving the room.

Increased Patient Comprehension

When patients who have already had a primary biopsy want to know why they need an additional procedure, Dr. Jensen shares digital slides in the PathologyWatch slide viewer to help raise confidence about moving forward with Mohs surgery.

Greater Office Efficiency

EMR integration saves the staff three hours a month chasing down reports and another seven hours a month importing the results. The digital slides allow staff members to quickly confirm suspected diagnoses and have a more profound clinical pathologic correlation.

Surgical Reassurance

Before performing a Mohs procedure, Dr. Jensen pulls up the histology and reviews the digital slide to make a decision based on what he sees and how the patient is presenting.

The practice, which opened in December of 2021, was founded by the guiding principle of “Putting Patients First.” Board-certified dermatologist Michael Lehrer, MD, FAAD, and his team specialize in skin cancer diagnosis, treatment, and prevention, including basal cell carcinoma, squamous cell carcinoma, and melanoma. The physician-owned dermatology clinic also treats various skin conditions, including eczema, hair loss, and psoriasis.

“Digital slides make it easy to access and see the specimen while discussing the case with a dermatopathologist or a patient. PathologyWatch helps us to provide our patients with the best care.”

—Michael Lehrer, MD, FAAD
Mesa, AZ
challenge

The Challenge

Before opening his practice, Dr. Lehrer previously worked in groups with longer turnaround times than PathologyWatch. If someone in the lab was away on vacation, the delay between biopsies and results could take a week or longer. Also, it could at times be challenging to visit the pathologist in person to discuss a particular case. Plus, relying on glass slides made it impossible to see the histologic sections on each case and establish accurate clinicopathological correlation on a regular basis.

The Solution

To follow his “Putting Patients First” truth, Dr. Lehrer knew he needed to review biopsies along side the dermatopathology results. By partnering with PathologyWatch, Dr. Lehrer has 24/7 access to digital slides, which he reviews on his desktop daily. The doctor also shares the digital slides with patients on his tablet to help describe their diagnoses and treatment plans.

At the same time, Dr. Lehrer has noticed an improvement in turnaround times, with PathologyWatch now returning most cases in one to two days. PathologyWatch can also turn around STAT cases in less than 24 hours in most cases.

challenge

The Challenge

On a typical day, the dermatologists at Advanced Dermatology send 15 to 20 biopsies to the lab. However, they waited up to 21 days for path reports to arrive. In addition to creating a bottleneck with office staffers, patients eager to learn their results and start treatment programs blamed the practice for the delays.

The Solution

PathologyWatch arranges for a courier to stop by the practice every day to ensure biopsy specimens reach the lab as quickly and safely as possible. After grossing the specimen, the lab prepares glass slides, which are used to create interactive digital images. Those digital slides are read remotely by academic-level dermatopathologists who return diagnosis to the practice in two to three days.

The Dermatology House is an award-winning medical practice in Park City, Utah, specializing in medical, surgical, and cosmetic dermatology. Board-certified dermatologist Kelly Stankiewicz, MD, FAAD, founded the solo practice in 2017 after spending half a decade in a larger multispecialty physician-owned practice.

“PathologyWatch has been a huge time saver. Simplified workflow, faster turnaround times, reduced human error, and clinicial/pathological correlation has helped us to maintain the high quality of care that our patients deserve.”

—Kelly Stankiewicz, MD, FAAD
challenge

The Challenge

Working with an out-of-state, hospital-owned lab introduced obstacles that complicated the workload for Dr. Stankiewicz and her staff. For instance, after going several months without receiving a bill, they were sent all of their invoices at one time, which created a very time-consuming and expensive roadblock. The lab’s pass-through billing process sometimes resulted in frustrated patients receiving double-billing charges. Plus, the lab only received samples twice a week, so turnaround time averaged between seven and 10 days.

While The Dermatology House focuses on the latest technology advancements, it did not have an integrated EMR. Instead, they had to segregate and organize stacks of faxes and manually upload information to the patient charts.

The Solution

Successful dermatology practices rely on dependable relationships and offerings from their labs. By partnering with PathologyWatch, The Dermatology House was able to increase efficiency with a state-of-the-art EMR interface that opened up a reliable digital workflow. PathologyWatch provides definitive case reads from academic-level pathologists, plus an account manager and support team at no additional cost.

The Challenge

Roman Bronfenbrener and his staff are committed to providing the best patient experience possible. But Dr. Bronfenbrener felt relying on glass slides would limit their ability to provide patients with the level of care they required.

With whole-slide imaging, Dr. Bronfenbrener can download specimens and easily zoom in on high-resolution images using his iPad, add notes, and share screenshots. Quick turnarounds are especially important for patients who are anxious to learn their test results and discuss their treatment options while viewing their diagnosis.
